Fees and Financial Policy

Thank you for choosing Dr. Fleming and Fleming Endodontics for your dental needs.

We understand that every person's financial situation is different. For this reason, we have worked hard to provide a variety of payment options so that you may receive the dental care you need and deserve that allows you to enjoy a healthy, beautiful smile with respect to your budget. Our policy is to clearly communicate these financial options to each patient in order to eliminate any confusion and to maintain a healthy patient-doctor relationship. Please feel free to ask us any questions about your financial arrangements as our mission is to ensure that you receive the highest level of dental care in a professional manner. Fees for endodontic therapy will vary based upon the particular tooth in need of treatment and the problem that tooth is having.

Payment in full is expected at the time the services are rendered. We are not contracted with any insurance company. However, as a courtesy to our patients, we will submit your insurance claim for you.

For our patients with dental insurance benefits, Fleming Endodontics will prepare and submit dental insurance claims. For your convenience, we can submit your insurance claim with the exceptions of Medicaid and Medicare. Because dental insurance companies will not divulge patient's personal insurance arrangements, Fleming Endodontics can only estimate benefits to be paid. For us to efficiently file your claim, please bring your insurance card. If your insurance policy does not have out of network benefits, Fleming Endodontics will be unable to file your claims for you. Please note that Dr. Fleming is not a part of your insurance company’s preferred provider list.

Our policy is to have our patients with dental insurance pay 50% of our cost at the time of your visit.* If your insurance company reimburses at a higher rate, we will refund the difference in the form of a check. Occasionally insurance companies will not cover the entire remaining balance; when this occurs you will be required to pay the balance. If insurance does not pay within 30 days, Fleming Endodontics will bill the full balance and ask that you collect the insurance funds that are due to you.

‘Reasonable Usual and Customary Fee’ is a term you may hear from your insurance company. The insurance company decides what they feel is a reasonable, usual and customary fee for the service based on demographics. This is usually considerably less than the dentist’s fees.

Patients without insurance will be required to pay in full at the time services are rendered.
We accept cash, Visa, MasterCard, Discover, and Amex. We are unable to accept personal checks. We also accept Care Credit. For more information or pre-approval of Care Credit, please contact 800-365-8295 or visit their website at www.carecredit.com.

You agree, in order for us to service our account or to collect any amounts you may owe, we may contact you by telephone numbers, which could result in charges to you. We may also contact you by sending text messages or e-mails, using any e-mail address you provide to us. Methods of contact may include using pre-recorded/artificial voice messages and/or use of an automatic dialing device, as applicable.

I have read this financial policy and agree to abide by this policy.
*Many Insurance Plans will only reimburse the patient for claims. Therefore, patients with these plans will be expected to pay in full at the time that services are rendered. Your claim will still be filed by our office as a courtesy.

A $100 charge may be billed to the existing patient’s account for any treatment appointment cancelled with less than 48 hours notice.
